 An evening has been arranged at Trent Bridge for you to get your CRB signed off before the start of the season. 

Date: Thursday 23^rd February 2012 Time: from 6pm to 8pm

Venue: New Board Room, 2^nd Floor, New Building (please see map attached) 

Please email chloe.gibson@nottsccc.co.uk if you will be attending the evening and a rough time when you think you will be there? 

To obtain a CRB form please contact TMG on 0115 9694606 and ask to speak to the ECB CRB department or go online http://www.tmgcrb.co.uk/default.asp?id=354 and order a form 

PLEASE NOTE WE WILL NOT HAVE ANY SPARE FORMS ON THE EVENING 

You will need to ensure the following is completed on the form:
- Completed using BLACK INK
- Complete section A-H in CAPITAL letters. Please write neatly, keeping letters inside each box
- Put a line through any mistakes and write the correction to the right. Do not use correction fluid.
- Ensure National insurance number is on the form
- On the back page write what position you are apply for a CRB check for and the name of the cricket club ignore section 58 on the back page
                Positions that ECB will CRB check: - Junior Cricket Coach, Junior Team Manager, Junior Team Manager, Cricket Scorer (juniors/open age), Cricket Umpire (juniors/open age), Club Welfare Officer, First Aider (juniors/open age)               

Bring with you: 3 of the following documents
·     Driving licence
·     Birth Certificate
·     Passport

Below must be within the last 3 months

    ·     Utility bill (electricity, gas, water, telephone, mobile phone contract/ bill
·     Credit card statement / Store card statement
·     Addressed payslip
·     Bank or Building Society statement
